Baby Care/Kids Care: Our agency is the Presumptive Eligibility site for Fremont and Custer County for the Medicaid Baby Care Program. Eligibility is determined by appointment. A Medicaid card is issued immediately, if found eligible. Pregnancy tests are also offered by the Agency.

Child Health Clinic: Child Health Clinics are held in our office two times a month. Infants six weeks to children through age 11 are eligible. Older children are seen for camp physicals. Our office DOES NOT give sports physicals. Payment is by sliding fee scale according to your income. No one will be denied services due to inability to pay.

CHP+: This program offers health and dental care coverage for uninsured Colorado children and youth. Applications for this program are available at our office. CHP+ also offers health care for pregnant women that do not qualify for Medicaid.

Communicable Disease: This office is responsible for the reporting and follow-up on communicable diseases in Fremont County. TB tests are given by appointment.

Developmental & Evaluation Clinic: The purpose of the clinic is to provide local comprehensive developmental evaluations to infants, preschoolers or elementary age children who demonstrate developmental delays who reside in Fremont, Custer or Chaffee Counties. Referrals can come from schools, parents, physicians, nurses or anyone working with children. A sliding fee scale for parents is used, or Medicaid or private insurance will be billed when appropriate.

Flu Clinics: Flu shot clinics are offered each fall throughout Fremont County. A nominal fee is charged to cover the cost of the vaccine.

Health Care Program for Children with Special Needs: HCP is a federal and state funded medical assistance program for children up to 21 years old that have a physical disability, which interferes with their normal growth and development. Our office provides case management for this program.

Home Health Care: A registered nurse, therapist and/or home health aide provide services to individuals in Fremont County in their own home. We bill Medicare, Medicaid, third party insurance and sliding scale for home health services.

Immunizations: Immunization Clinics are held the first Wednesday and fourth Monday of each month. Both clinics are by appointment. Age appropriate vaccines are administered beginning at two months of age. TD (Tetanus) is available for anyone over the age of seven. There is a $2.00 fee charged for each immunization. No one is denied services due to inability to pay. Immunizations are also given on a drop-in basis by arrangement.

Other: Nurses are available to check blood pressures or ears (if a child is ill) on a drop-in basis. For these services, nurses are usually available between 8 a.m. - 9 a.m., Monday through Friday. The office also has resource material available to give or loan on a variety of topics. Nurses are available to present educational programs to the public. Our office is actively involved in the Emergency Preparedness Plan for Fremont County. We are also involved in West Nile Virus prevention and follow-up.

Prenatal Plus Program: This program provides prenatal education in group, as well as individual settings. Classes are available for pregnant teens at local high schools. This program is available under Medicaid, CHP+ and some insurance plans. However, all pregnant women are welcome.

Tobacco: Provides information, resources and referrals to adults and teens for cessation services. Quit Kits are available free of charge. Quit Kits are available for adults and teens for inhaled products, chew tobacco products, pregnancy specific, male and female specific. The Tobacco Coordinator is available to answer questions and concerns related to tobacco use. Funds are provided for this service through the Master Settlement Act.

Well Adult Clinic: Well adult clinics are held monthly at the Garden Park Villa, the Royal Gorge Manor and Coaldale. Individuals must be over 60 to receive this service. There is no charge for this service, but a donation is requested.
